CANCELLATION, NO-SHOW, AND LATE APPOINTMENT POLICY

Initial evaluations booked online must be cancelled 48 hours before the appointment. This is to allow all clients to have an equal opportunity at having convenient appointment times, so we greatly appreciates giving us notice. All other appointments must be cancelled at least 24-hours in advance, or you will be charged 50% of your appointment cost. This ensures that all patients have fair access to the appointment times that work best for their schedules. Please arrive on time to your appointment. We do not extend your appointment time if you arrive late, as it would take time away from other patients.

GOOD FAITH ESTIMATE

It’s important for you to know that you have the right to receive a “Good Faith Estimate” outlining how much your medical care is expected to cost. This notice is required by law for patients who don’t have or who aren’t using insurance for medical items or services, and must be provided in writing at least 1 business day prior to your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service or item. Be sure to save a copy or photo of your Good Faith Estimate. If your Good Faith Estimate is $400 or more under the bill you receive, you can dispute the bill.
